    I visited this dental practice with a broken tooth on January 10, 2024. My dental insurance was expected to pay 100% of the cost of repair but the dental office required I pay $1,872.00 up front. They agreed to submit immediately to my insurance carrier. It took many weeks of my calling them before they finally filed with my insurance and the Explanation of Benefits came back showing I only owed $25.80. I made several requests for refund of the overpayment before finally getting a call on 3/6/24 that I could go pick up a check. The check was short $84.00. ***** is the employee who files insurance and she told me she would discuss the short pay with ****************, the dentist, and get back with me. On March 8, 2024 ***** called me back to say she was putting the check for missing money in the mail. I never received that check for $84.00.On April 5, 2024 I mailed a letter (attached) asking for the missing $84.00. To date I have not heard from the dental office and have not received the $84.00 they owe me.

    Insurance companies do not pay for dental products.  They had no right to guess what a certain product would cost.  My reimbursement did not even cover the cost of the product.  The patient told me she agreed to pay me for the products, so that is where the discrepancy came in.  In any case, I have sent her a check, a copy of which is attached here.   I send refund checks at the end of each month.  I received the patient letter in April.  Thank you.

    Regarding claim no. ******** against ****************. His last response to you was that he mailed me a check in the amount of $84.00 on 4/23/24. It has been two weeks and the check never arrived. His staff has been promising to return my money since January and it never happens.

    I went into Belvedere Plaza Dental for annual exam and cleaning, and I was told that there was all of this work that needed to be completed. I allowed **************** to place a crown and the cost was much, much more than I have ever paid for this service, and I have three dental insurances. After paying $225.00 out-of-pocket, I reviewed my processed claims and noticed that my Dental Insurance Companies had paid the for services in full. I asked the dental office about that I received some general answer. I do not believe that the Belvedere Plaza Dental is trying to take my money and do me a disservice, but I do think that they had an issue with patient files and required payments. I think I was their first patient with three Dental Insurance Plans.

    ************** has 3 insurances.  We always accommodate all our patients.   We filed our services with all 3 of **************' insurances. 

    As you can see in his attached ledger, I wrote off quite a bit to accommodate the patient and abide by his various insurance plans.  I do not see where any errors were made.  As you can see in his Ledger, all payments for services have been accounted for and write-offs completed based on insurance explanation of benefits.  The patient never voiced any complaint to us and if he had, we would have been more than happy to explain his ledger, insurance benefits and payment for services rendered.  He came into our office and asked for his records stating he would rather visit a Dentist who accepts all his insurances.  Unfortunately, we are not Providers for his main insurance, which is Cigna.  I hope this helps.  If you have any further questions, I will be more than happy to clarify.  You may contact me directly on my cell phone at ************.
