            I left the care of CVC some time ago and went to another Doctor. I notified CVC that I will no longer be using any of their services. (They have a record of this) I have a home device that is to monitor and transmit my function of my heart. Recently I received a phone call from a collection company stating I owed CVC an unpaid debt and demanding payment. I called CVC and was told it was the insurance balance due from me for current monitoring, review, and reporting. I explained to them I have never received any reports or notification from them. I also stated I have never seen any billing. During the time I used their practice, all communications were done either email or phone. They agreed they had no record of reporting to me results of being monitored and they had sent the **** to an old address which I had not lived in for some time. They did submit a **** to ******** and was paid. They admitted they had my phone number and email but never attempted to contact me claiming they were short staffed. Instead, the simply sent the **** ($50) to a debt collector and this charge is now showing up on my credit report. I am fully disabled with a heart condition and since the time I left CVC I have had three additional heart operations under my new doctor. I if in fact any monitoring had been done, I might have been able to avoid those operations. Can you please help.
