    On February 27, 2024, I had a routine doctor's visit with NP ***************************** ******. I am a cash-pay patient, but need a copy of a bill for reimbursement purposes, which I made clear at the front desk.On the date of the appointment, I received (separately) a receipt and a statement of what was provided. The statement was insufficient for reimbursement purposes, as the receipt and statement are entirely separate documents with no attestation to link them together.Since then, I have reached out three times by phone to the doctor's ****** to get an invoice or bill sent to me. On the third call, I was directed to the billing department, where I was told I had an outstanding balance of about 60 dollars. I am a long-time patient at the practice, so there is a chance this is legitimate, but the agent on the phone seemed to be confused whether the balance was from 2024 or 2023.Not only am I waiting on reimbursement, but either these charges are for old services I never received a bill for (and which I would be happy to pay, but it means something slipped through the cracks on their end as they had been sending me bills when I hadn't paid in ****** in the past), or there were uncommunicated charges following my in-****** visit.At this point, I just want a clear billing statement which I can submit for reimbursement.

    I visited *********************** on Feb 27 for a Wellness exam. I chose her because she is in Network for my health insurance. I never received a bill from her for this service, but I recently got a bill from a debt collector for $414. After much research I have learned that my insurance denied her claim of $414 and stated they would cover only $146 not the $414 that she charged (for a wellness visit?). She a PA not a doctor. I didn't realize this as I never received an invoice from her. I have received a invoices from her office for other visits so I know the office has my correct address. When I called the office they tried to connect me to their billing **** but that line was disconnected. I called back and get in touch with a call center in ***** and couldn't understand the lady, but she said they cannot send me an invoice because they had already. I shared again, that when I get an invoice, I pay it right away and I had not received any. She was not helpful. So I called my insurance company and they explained the situation. My concern is that the cost of the exam was such a surprise, and I was never advised of it. Did they intentionally not send an invoice because of the cost? I selected *********************** because she was 'In Network" and there would be contracted rates for services. I didn't realize she can change those rates and charge what she wants. But she does have to send an invoice. I'm pretty sure of that. And their lack of professionalism in not sending an invoice and sending me to collections. I am just so disappointed and angry at this so called 'doctors office'. I have sent the debt collector $146 for the agreed amount for the services but dont' think I should have gone through the stress of dealing with this, and with a doctors office so clueless to explain or help solve. Its crazy. The Debt Collector is Plus4Inc.

    on 4/14/22 I went to an appt with my pcp at the 4c Med Group office on **********- ******** ** office. ***** office clerk told me there would be a $45 copay required. they placed a $45 charge on my **** card. when I got home I glanced at my BCBSAZ insurance card and realized there should be 0 (zero copay) to see my pcp. I made attempts to call their office, but could never get thru. I called BCBS member services for help advising 4C med group of the error. (BCBS confirmed I should not have been charged a co-pay). ***** at BCBS kept in touch with me regarding her repeated attempts to reach someone at 4C Med Group to advise of the billing error. On 6/16/22 I tried calling 4c Med *************) to speak with office mgr (********). she was there, but did not come on the line. she told the person who took the call to just tell me "she was not handling this refund any longer because she sent co-pay refund info to their vendor (Unislink) who handles refunding." 4C rep I was speaking to told me to call Unislink *************) to inquire about refund status. The rep at Unislink told me "Unislink cannot make a credit card refund. a refund check from 4C Med Group must be issued." (so I just experienced a total run-around) Unislink rep told me he would have his manager call me back--I never received a return call. on 6/27/22 I faxed an ultimatum directly to ******** (Offc Mgr) that if I didnt receive this refund within 7days, I will open a case with BBB. she called back and left voice msg on 6/29/22 that "Unislink is mailing me a refund check for $45-- I should receive it in about 7 days". It is now July 9th, and I have not received any refund. this has been kicked around by 4C Med Group from 4/14/22 to 7/9/22 with no resolution. they made the error in billing me-- they owe me $45

    Have not been able to reach the medical office/provider for the past 2 days for lab results. Voicemail prompt states the office is closed and to call back during business hours, but call times were during regular business days (6/1 and 6/2).
