    I had an appointment on 10/01/2024 at 12:30pm and upon a urine test at this facility I had blood in my urine and need an ultrasound done as well. Anyways, I provided the exact ad**ess of the preferred ********* pharmacy in ******** to use for the antibiotic prescribed and they sent my prescription to a whole other city! I also requested a ** note due to leaving work early 10/01 for the ** appointment and needing the next day off as I'm in a lot of pain. They told me they would put in a message for the ** to get it out to me in 24 to 48 hours! What? I needed it for yesterday but have to wait for the next day or possibly 2 days? Makes no sense. I went back 10/02/2024, again left early from work due to pain and not being able to walk and to let the ********* know about the pharmacy not having my prescription and they again said they would send a message/call within 24 to 48 hours! Come 10/02/2024 08:34pm, still don't have a message or call from the *********. I called the ********* again and they just say they will send a message to the **. ***** the pharmacy still has not received my prescription either! I'm still in pain and lost 2 days of taking the antibiotics going on 3 as I work at 5am and possibly being able to work because they don't have a sense of urgency to get to this I guess. Will definitely look for a different urology place. Very angry, frustrated, and disappointed with this "health"care facility.

    My husband, ****, went for a vasectomy consultation in January. He was told he had to pay $500 upfront, even though our insurance fully covers the procedure. He paid in January. He was told he could request the refund of $500 (minus whatever copay we'd have to pay) 30 days AFTER his follow up appointment. His surgery was in March, follow up in May. He requested the refund June 5th. He calls June 25th to get the status of the check as we had not received it yet. They said it was never cut and "didn't know why it wasn't cut". So they said they see it was cut on June 24th and it should only take a few days to get. He calls them 2 weeks later because we still hadn't received it, they told him they would cancel the check and cut a new one. He calls back last week and they said it was never cancelled "because they didn't know if we'd cash the old check and then the new one as well" indicating we would steal from them by cashing 2 checks. They then said they could cut us a new check when the accountant gets in, but they only work Thursday so if the account doesn't get to it it would get pushed another week. We're now almost 2 months from when we requested the check and have yet to get a check. Every time we call they give us the run around. We left a message with the supervisor ******** last week and she never returned our call. When we asked another employee why she hadn't returned our call, the employee said that maybe "she heard our voicemail and figured why call back when they would just send another check out." What kind of customer service is that? We are owed roughly $450, after our copay is deducted. Every time we call the billing department they give us a different reason as to why we haven't gotten our check or why they haven't sent the check or why the check wasn't cancelled and a new one cut. This is extremely shady. They are now holding my money hostage and will not give us our money that they should have never required upfront to begin with.

    On 11-11-2021 I had an appointment with ************************, I spoke with ******************************************* m.a. for approximately 30 minutes (which might have been longer) and answered many questions. I needed to go to the ** and asked if a UA was needed she said yes, I asked does it need to be a clean catch, and the ma said no just urinate in a provided cup and put your name on it. I was not informed it was "special UA" that costs about $3,700.00 of which I am being billed $1050.00. Spoke with ************************ for maybe ************************************************ and they also billed for a bladder Ultrasound which was not performed, supposedly it was a wand waved over my bladder to see how much urine remained. Paid $50.00 for the **** forms that I left in the office, they sent in the wrong forms. Called approximately 16 times rude staff, asked for Manager spoke with ***** she said she would handle it. Nothing has been fixed and they are still billing me & never got my $50.00 back for their error. OPM.gov for complaint also.
