    March 5, 2024, Saw Dentist (Dr. Azadeh Amini DMD or Abelardo H Valdes, DDS). Set up what procedures will happen and time frames. 03/18 & 30 & 04/01, first forms made of teeth/gums for dentures. 04/17& 18, teeth pulled, new dentures fitted. 04/30, fitting and check gums. 05/13, 21, 28, 07/02, 08/12, 20, 09/16, 10/02, all fitting appointments. 10/02 Appointment to discuss permanent dentures. (Have an overbite-have addressed this with Dentist and Dental fitter, every appointment-one meeting advised did not care what they looked like wanted to be able to chew.) Dentures were extended in front to hide underbite which made the teeth in front extend out like buck teeth, also made my mustache push up. The dentures for fitting fell out when I talked and no chewing food at any time. I was told by Dental person they could not fit me with dentures, I would have to go see another dentist. I tried to find a dentist that my Medicare Ins would accept. No dentists would work-most would not take Medicare Ins, others wanted $$$$ to cover charges if my insurance would not accept them as part of plan. I am 81, loss of teeth has caused 30 lbs. of weight loss, intestrial problems (Daily-now at 2 pills a day), affected me mentally with depression and will to move forward. I am embarrassed with my speech daily. Talking and chewing without teeth has created a major health and communication problem for me. This dentist said he could fix my teeth issues and failed to listen to my concerns and turned his back on me when I complained about the dentures not fitting and why they were ill fitting. I want dentures that stay in, I can chew solid food with after 6 months and not having the dentures fall out when I speak. I WANT MY TEETH/DENTURES TO FIT MY JAW, MY MOUYH, MY JAWLINE,! I WANT TO BE ABLE TO CHEW FOOD & TALK WITHOU DENTURES FALLING OUT! I WANT WHAT WAS TOLD I WOULD RECEIVE & BE ABLE TO EAT AGAIN!

    My name is ******* **** and I went to Santa Cruz River Dental because they said they were part of the ppo plan from my Detail insurance plan so I ask for a predetermined price witch they didn't do so they changed me what they won'ted to so in stand of getting my insurance company involved i pade cash for all the work they did to my mouth I asked if this will take care of everything they said yes it will but they changed my insurance company when they said i had took care of everything and said what the insurance don't pay i have to and I already paid with cash they gave me a reset for the money but I can't understand what they did no dollar amount is on it so you can't tell what i paid really and the phone number on the bill is not Santa Cruz River Dental it's AZ Dental 520-547-2328 it's not that much money they say i owe it how they are going about trying to get it lien about it i made sure I pade very dime they said I owed and more and they never gave me my change back they would always say they have no change i hope this is enough to file my complete what they are doing is not Right May GOD Bless you all at the BBB
