    Im a 69 year old Type-1 "brittle" diabetic in bad health that relies on CGM sensors and insulin to stay alive. Ever since I started going to CSV I have had problems with them in getting my sensor orders and presciptions filled in a timely matter. This has put me at risk of death. In the last few weeks things have just got worse. Their refusal to respond to what my sensor supplier (********) requested resulted in a long delay that caused me to run out of sensors (again, not the 1st time this has happened). And now they are not acting on multiple requests from Express-Scripts to authorize my insulin presciptions. Rx# ************ for ****** is the most urgent presciption I need to have filled, and they have delayed acting on it for weeks now.

    After further review, there is insufficient evidence to substantiate the individual's claim. Regarding the patient's ****** Insulin prescription, a refill for this medication was dispensed August 31st, 2024. In addition, the patient has been prescribed an additional refill for this prescription on September 16th, 2024, following provider evaluation. In addition, the patient states that CSV has been unable to prescribe sensor strips. To obtain this prescription the patient would need to be evaluated by a provider. As a result, an evaluation was scheduled August 29th, 2024, determining the quantity of sensors that would ensure the patient's long-term health. However, the patient was unable to attend this visit and as such a long-term care plan could not be established. Per our compliance and risk manager, the patient was prescribed a short supply of glucose sensors to ensure the patient's healthcare concerns are properly accommodated until re-evaluation can occur. The patient was re-evaluated September 16th, 2024, and was ordered an additional supply of glucose monitoring strips.

    I went in for an examination for a cut/gash on my finger, that I thought needed stitches, was informed by attending physician, that it was too late for stitches and after 24 hours after incident. Physician said she wanted to give me a tetanus shot, i asked it was optional, and she said it was but suggested i should get the tetanus injection, I replied, "NO" because I could not afford it. She then went to see if was covered so no payment would be required for tetanus injection, Well today I received a bill for the vaccine and the administering the shot as well to the amount of $108.

    In February of 2023 I began my attempts to receive a refund check from Clinica Sierra Vista due to the business failing to apply my medical and charging me out of pocket for medical visits . From what I can access through my online account the charges included 4 $20.00 payments made in 2022 on dates : Sep 7 , Oct 19, Nov 13, and Dec 12. I have contacted Clinica Sierra Vistas billing and finance departments multiple times throughout the past 10 months regarding this refund check that I should have received at the end of February 2023 according to one of their own staff members . Every time I call this business, nobody can offer me answers on where this refund check is. They claim theyve sent it to finance and finance wont get back to them about it. They claim theyve mailed it out. They claimed its gotten lost and had to be re-submitted on their end. Their story is constantly changing as to the status of my refund check. Multiple employees in the billing department and finance department have confirmed that I am owed money from their end, yet I have yet to receive it. *** tried to go through Clinica Sierra Vista directly for months, but they refuse to give me clear answers on my money. They also refuse to send the check out and continue to blame either finance / billing for not getting it out and mailed successfully. Ive had 0 success getting any answers from this business and its almost been a year .

    I first had a walk in appointment with Dr ***** ****** FNP-C on Tuesday April 4th,2023 to address the following issues (Chronic left -sided lower back pain and chronic hiccoughs). Dr ***** ****** then scheduled me an appointment the next day to have X-rays (4) images performed. Then I was told that I have to see Dr. ***** because she was my primary doctor so On April 11th,2023 I was seen by Dr. ******* ***** FNP-BC, for the same issue of Chronic left-sided lower back pain with left-sided sciatica. Then on May 5th,2023 I returned to see Dr. ***** so that she could fill out my short term disability paperwork from Edd, Form (DE2525XX) and submit back to EDD via Online and she did. However when Dr. ***** submitted the form she put in the wrong (CED) of 05/03/2023 claim effective date. With the (CED) date being wrong my Edd claim has been returned to me and will not be processed until the (CED) of 04/11/2023 the date I was seen by her date is corrected onto a new form called a 2547D. EDD and I have both sent them the form 2547D to have updated but I have heard nothing but excuses as to why it's not being filled out and sent back Edd I need this taken care of. So with that I am in need of help getting the doctors to submit forms to the EDD department for my short term disability. The EDD has faxed clinica sierra vista forms to pertaining to this matter, but they've not responded back. I have contacted clinica sierra vista several times in the month of May 15th 2023 until June 14th 2023 about this matter just to have it fall on deaf ears. Dr. ******* ***** informed me on our phone appointment June,9,2023 that she was going to take care of the issue in five days...it never happened. Dr. ***** ****** also has a Form 2547D that he need to fill out as well with the date 04/04/2023 the day I was first seen by Dr ***** ******. Can you also remind them to fill out the paperwork correct because that last paperwork was not. Thank you ****** ****** *r. I have after visit summaries.
