    For physical therapy services received by Movement For Life at **** * **** ****** Rd [###-###-####] on Oct 23, 2023 I was later charged $94 six months later (April 2024) due to an error by that office in their billing / deductibles assessments as confirmed by my insurance (Premera). The front office manager "B*******" from the above Tucson address stated that a new plan was in place and "I guess it wasn't posted" -- this ambiguity and lack of confirmation on behalf of a billing front desk does not seem fair, especially to the consumer. There are further discrepancies that seem unfair. I also visited Movement for Life in Arizona on October 31, 2023 and November 7, 2023 and from both occasions was later charged an additional $97.60 in mid December 2023 apparently due to similar lack of confirmation between Movement for Life and my insurance. It does not seem reasonable to provide consumers indications that their bills are settled upon paying a deductible and leaving the physical office and then to receive multiple bills several months later. This practice of being potentially liable to unlimited bills several months' after services received should likely be addressed. Note: in the above attachment, the Movement for life address is listed in San Luis Obispo, but the physical services were in Tucson, AZ.

    As a patient receiving treatment for a neck and shoulder injury, I was told directly by the office manager of Movement For Life Physical Therapy that I would not be charged for services received. When I was billed $150, I called the business and was told that the office manager would contact me. The office manager did not contact me so I called the billing office letting them know that I had not received a call. The billing office was very understanding and let me know that the office manager would soon call me which he did. The office manager apologized for not contacting me and admitted that he was mistaken in telling me that I would not be charged but that there was nothing he could do about it. Had I been told that I would be charged for the January 2nd and January 4th PT treatments, I would have cancelled those two sessions. Repeatedly, the office manager admitted that he made a mistake and that he realizes that it is unethical to tell someone there is no charge and then to charge them. He added that the $150 fee could be waived in some cases but not this one though his explanation was vague for why telling someone there is no fee and then charging them is not one of the reasons why the fee could not be waived. The bottom line is that I feel I should not be responsible for paying for appointments that I would not have accepted had I been informed there would be a cost to pay.     On my initial visit, Movement for Life gave me a good faith estimate of my part that would be due for services after insurance, which was $0. After my last visit, they sent me a bill for "my part" of the balance due. After talking with Angie, she informed me that the person filling out the initial form failed to completely fill it out. At no time during all my visits did anyone tell me I needed to pay a co-pay or anything else. They had numerous opportunities during my many visits to let me know of their mistake but they did not. I would not have done any physical therapy at all if would have known that I was to incur any cost to myself whatsoever. I feel like this could be a strategy of theirs to create visits from patients who unknowingly think they will not have any charges.
