  My daughter went to the the dentist on 12/19/2022. At the time I didn't know she wasn't on my insurance because she aged out and was attending college and I was unaware I had to submit documents to keep he active. The amount owed was $735. This was sent to collections. After I had my daughter added to my insurance the claim was resent. The dentist office billed $687 for 6 services provided. I received another collection letter yesterday and since then the bill has escalated to $1,018. There are new charges for new services that were never submitted to my insurance. How am I sent to collections for a payment on a service that was never submitted to my ***************** Of the original $687 my insurance paid $487 and $200 is owed by me. This recent bill says I owe $583. Most likely because there are new charges on this bill that were never submitted to my insurance. I have been going back and forth with ********************* whi never follows through, nevercalls back like she says she will and had been dishonest and passive about this whole issue.

  In the past I have had services completed and have not been satisfied. I have paid over $3000, and have received some of my refund, but not all. I have contacted Clermont Smiles on numerous occasions and still not have heard back from them and my complaint has not been resolved. I have spoken to an employee by the name of Madison who have helped me receive partial my refund but is no longer working there. I have spoken with the new Accounts Manager by the name of Angela, who has not been helpful at all. I have attached all payments that I have made, and my insurance company has made and still, there is a balance that the company cannot account for. I also have attached the refund that the dentist office has refunded back to me. I am really exhausted in contacting them and there has been no realistic response. I will like a full refund. My sister used her credit account to help pay for my services. Attached are the receipts that I have.
