    Attached are the following documents attesting to the fact that this individual is making false accusations and trying to extort the Doctor's Practice.
    Case #********** opened January 13, 2025, with United Healthcare agent Cath F phone reference ending in #**** attesting this patient has a daily Copay fee of $20. This information can be verified by calling ********************************** at ************ and referencing the above case #.
    Attached is United Healthcare Website Eligibility Details Page #9 Highlighting a Specialist Copay Visit Fee of $20 verified by the ***************** agent Cath F January 13, 2025.
    Attached are United Healthcare Cards #1 and #2 attesting a specialist copay of $20 and verified by ***************** agent Cath F January 13, 2025.
    Attached are Outstanding and Past Due Invoices for Services Rendered totaling $100 for outstanding copays that the Patient Refuses to pay.
    On 12/14/2024, the office received a RX Request for an annual renewal for medication required by the patient. ********************************* the patient and notified her that they needed to be seen by the doctor for an update to the medication and to be seen the Patient needed to pay up their outstanding fees of $100.
    On 12/18/24 the doctor called the patient. To accommodate the patients needs the doctor allowed one month RX renewal for the patient to have medication and required. The doctor ordered the patient to make an appointment to be seen. And to pay their past due balance on their account. Further the doctor agreed to waive the past due COPAYs as a gesture by working with the patient with the agreement that in moving forward, the patient will comply with their Insurance Policy. The patient agreed and made an appointment for 1/08/2025.
    On 1/7/25 the patient came into the Practice and cancelled their appointment for 1/8/25 and said they had made an appointment with another doctor. Then the patient demanded to be paid monies and left with threatening remarks disrespectful to a patient within a doctors office.

    I am rejecting this response because: Eye See Clear's records show that my insurance pays them in full for my visits. I have attached their records showing all amounts due are paid in full. The record dated 4-23-24 shows a credit to me of $13.56 due to my paying them a $20 co-pay. Which is not actually correct per my attached evidence of benefits. The amount allowed per insurance for an office visit is $75.06 not $80 (which I tried to explain, but he did not understand), minus the amount paid by insurance in the amount of $73.56 leaves a credit balance of $18.50. There is also a record from 7/23 that I paid a co-pay showing a credit of $20 that they refuse to print out for me. They keep insisting I owe a co-pay for every visit which would only create a $20 credit for each visit. They do not understand medical accounting. I suggest that any of their patients check their accounting against the evidence of benefits from their insurance. Furthermore, I called them on 1-7-25  and did not make a seen at their office. It's the principle that they will not listen to someone who has an accounting background. They must really need the money from a retired fixed income elderly person.

    I am rejecting this response because: I still disagree. For some reason all my files did not attach with my last response. I will try to attach them again. I did return to my previous doctor and had a visit this week with no $20 co-pay. Not all insurance is the same. I also talked with ***************** and they told me to refer to my evidence of benefits which show my share is $0. This office will never listen to reason.

    Uses wrong insurance for guests and charges more money than should

    This Complaint Provided no information on their "Patient Registration Form" with regards to any insurance Information (attached) 
    This Complaint provided their Insurance card (attached) 
    The insurance information was submitted, and the patient was advised of the charges. (attached) 
    The patient was belligerent and condescending to the Staff and the Owners of a professional **************  
    There is No Merit to this complaint other than the fact than the Complaint wanted to pay nothing for services rendered. The doctor does not provide services for free.
