On November 27, 2023,our client brought her dog for a medical checkup. According to her the dog was vomiting non stopping since the night before, was inappetence, and not urinating. The patient was indeed depressed, dehydrated, with skin lesions, vomiting, and abdominal pain. The client was presented with different options to treat her pet.  The pet was treated with antiemetic and iv fluids. After a long discussion with the owner about all the possibilities to treat and help her dog to feel better, she agrees to have an x ray without a radiologist interpretation and after the x rays and treatment the pet continues to vomit in clinic. Ultrasound, CT scan was recommended because due to the pet conditions and symptoms the doctor suspected a mechanical obstruction. The owner declined all other tests (Blood work) recommended. Also, it was advised to take the pet to an emergency clinic, but the owner expressed her concerns about costs. The doctor proposed an exploratory laparotomy procedure, which the owner agreed to. During the procedure there were various findings and anomalies in the digestive system and was determined that the pet has paralytic ileus by a nonspecific cause.  After the procedure the owner was advised to take the pet to an emergency clinic for 24-hour observation and to continue post-operative treatment, since we are not a 24 hours clinic. The pet was sent to the nearest emergency clinic, which is the normal method after a procedure that needs more care.
