    We took our two minor children (****** and ***********************) into Dr **********************'s office for a routine annual dental exam on 11/11/21 and routine dental cleaning on 12/23/21, both of which are 100% covered by our dental insurance plan. However, we found out about a year later when we received a bill in Nov, 2022 for $520 and copies of insurance claims (submitted on 10/20/22) from Dr ********************** that they just submitted claims for additional services (multiple x-rays and 12 sealants for each child) that they allegedly performed during the visits in 2021 without providing us a treatment plan, estimated cost, and obtaining our consent to these treatments/costs. When we received the bill (attached), we called them to get clarification and a copy of the treatment plans. They emailed us on 11/17/22 (attached), and their explanation for the additional treatments and charges were that Professional opinions from Doctor ********************** and ******** the hygienist were to apply sealants for their oral health and the best treatment for your children.. They were unable to provide a treatment plan or proof that we consented to these treatments; they only sent us a copy of the New Patient Registration Forms that we filled out on 11/11/21 (attached) for the childrens dental exam. We responded to their email (attached) stating our concerns about these unapproved treatments and charges, and we asked them to write off the costs. We havent received a response/resolution, but we got a new bill via mail about another year later, on 11/30/23 for the same charges (attached). We feel wronged and deceived by Dr ********************** and his staff for charging us for additional/medically unnecessary treatments for our children that we were not aware of and did not approve. Also, when we requested a copy of our childrens x-rays, they did not provide an x-ray of our son; they sent us an x-ray of a different person called *******************************, which is a HIPAA violation of sharing a patients' protected health information.
