    The main reason I'm leaving *****. I've called numerous ************ been treated poorly by the representatives. Same is true for Nations Benefits, the company who distributed the grocery allowance. Who thinks it's appropriate to scam the elderly? I guess we're easy marks.

    I have a different complaint. I applied for a job there, and in their online application for employment, they wanted my date of birth, SS#, and whether I was between the ages of 18 and 40.I did not fill the answers out, and subsequently I was contacted by email from an HR *** asking that I complete the application.Federal laws give applicants, and anybody for that matter, the right to refuse SS#s. This is inappropriate for an initial job application, and only appropriate at the time of hire or if incumbent agrees to a background ********** of birth is inappropriate for an HR application. Narrowing age range suggests the possibility of covert age discrimination for applicants over 40. How clever to phrase the question that way as it is appropriate to ask if applicant simply is over 18 years of age.I contacted the HR *** about this, and have not received a reply. Res **** loquitur-it speaks for itself.Think twice before you apply to a company like this, especially in view of the BBBs F rating.

    Its time for me to leave this plan. They dont have many dental office in network, so I have to travel an hour away to be seen. Also this plan doesnt cover ANYTHING! Im so dissapointed because this insurance is offered to low incomed individuals, and we are being handed $2k-3k bills from our dental offices to fix simple issues. Ive been to two offices and im over it. I have to leave Liberty.

    We were lured by advertisements into switching from Medicare to an ***** Medicare Advantage plan by the ***** get money back every month for groceries and pharmacy products. We were especially interested in the Dental Insurance which we later learned that payments are handled by Liberty Dental. Our disabled son was assured by the dentist that Aetna covered all services provided and after extractions and cleanings ***** was billed roughly $2,000.00. Because we were unhappy with ***** all the way around, we cancelled in early 2004 and went back to regular Medicare. TO DATE: *****/Liberty Dental have paid NOTHING toward the $2000.00 Bill and the services were provided in February and March 2003. Expect this incompetence if you sign up with either company.... BeWare....

    I would not give any star. I have an emergency and in pain. I need an Endocrinologist for 2 weeks now. I have called every in network physicians. They either no longer accept the insurance or the provider no longer there. One near by went on vacation.I called Liberty. They gave made an appointment for me which is the same physician who went on vacation. I have to wait almost 3 weeks for the appointment. I can't go cause I have another procedure done. After waiting two days for filing grievance. No one called back. I called them back now they said they closed my case. What a waste of time! They are whole bunch of useless people and this is the worst company to have dental insurance with.

    It is a crappy "insurance" company. ** HMO has them listed as a dental provider. My mother has no bone on her two front teeth and they need to be extracted. Three weeks later AFTER the pre-authorization request was submitted, they are still deciding. Don't deal with this "dental care insurer." They want the money but don't want to provide health care.

    I’ve had unbelievable problems finding an oral surgeon. They sent me a list and none I call accept their insurance . Had to get my own and pay out of pocket. Going to try and get reimbursed. Crossing fingers. Spent three months and hours on the phone to no avail. I’m in ruskin area. Liberty set up an appointment and after a half hour drive they told me they don’t except liberty. I’m at wits end. List of providers they sent me the first five on their list DONT EXCEPT LIBERTY!” They have problems getting oral surgeons.

    Liberty Dental Plan of Florida Insurance:
    CGH, who is a resident at Oak Manor Skilled Nursing Facility, has had 12 visits to your dental offices, first at Tyrone Mall Dental P.A. and then at Countryside Mall Dental P.A.
    Final dentures were too small and lacked molars.
    Those visits incurred travel costs of $335.25.
    Since she has not received useable full upper and lower dentures after all this time, she has no choice but to seek dental care elsewhere. As you know her current dentures, which are over 20 years old, are barely usable.
    As consideration for the inconvenience and inability to provide adequate dentures over the past 15 months, reimburse for travel expenses in the amount of $335.25 is a reasonable request. A list of visit dates and transportation cost is attached.
