    I went to Polaris Spine and Neurosurgery Center in *******, ** for a hospital follow up on 3/14/2022. During this visit, the physician (*********************************) recommended injections in my back to see if I would get some pain relief. He said that I would need to schedule this in order to give the insurance company (United Healthcare) time to approve the procedure. I told him that I had the funds to pay for the procedure in full ($3,745) if they could fit me into the schedule today. The practice made space on the schedule for this day. Everyone understood that I was paying out-of-pocket for the procedure and I would personally file a claim with United Healthcare afterwards for reimbursement. I had a follow up visit from these injections where they scheduled me for more injections. As a means of getting on the schedule ASAP, I told them I would again pay out-of-pocket and I would personally file a claim with the insurance company for reimbursement. They charged me the Self Pay Rate of $460, per the billing employee who called me just prior to the injection date to take my payment, which I paid in full. I later reached out to United Healthcare to file my claims but found out that Polaris Spine and Neurosurgery Center had already filed claims with the insurance company in which the insurance company had already sent them the payment. The insurance company told me to reach back out to Polaris regarding my overpayment to them. I have reached out to Polaris on several occasions reviewing the situation and requested they correct the overpayment and reimburse me the monies owed since they took full payment from me up front AND afterwards filed a claim with the insurance company and accepted that payment also. Other than the billing employee escalating up to her supervisor, there has been no resolution or communication back from Polaris regarding this. Additionally, they have sent me a **** requesting payment and on the ****, it does not even show where I paid previous
