    After going on ******** (March 2022) I have not received CPAP supplies from CPAP Medical. CPAP Med has told me they have not received the required documentation from Gwinnett ******************************* I contacted Gwinnett Pulmonary (hereafter named GP to save space) multiple ************ have not helped to provide the required documentation to CPAP Med.- 8/29/22 - called CPAP Med asking why I hadn't received CPAP supplies. They stated they had not received office notes from my doctor - 9/7/22 - called at 1:27pm & left voice message, call not returned by GP - 9/9/22 - called at 9:51am & left voice message, call not returned by GP - 9/12/22 - email from CPAP Med stating they also contacted GP requesting the documentation but have not received the documentation - 9/13/22 - visited GP at 1:30pm & was told the business office was closed Friday afternoons. Receptionist took my information & ensured me she would have them contact me to assist. No callback was received - 9/18/22 - filled out GP web site request form for them to contact CPAP Med & provide the information required - 9/18/22 - received message from GP that they have faxed over the 2018 sleep study to CPAP Med - 11/16/22 - appointment with *************** & explained that GP needs to contact CPAP Med, determine what information is missing, & provide the information - 11/19/22 - email to CPAP Med asking if they had been contacted by GP & provided the required information. CPAP Med replied back on 11/30/22 informing me they haven't received anything from GP - 12/16/22 - filled out GP web request form asking for assistance. No reply to date The assistance I need is:- GP CONTACT CPAP MED TO FIND OUT WHAT DOCUMENTATION IS REQUIRED - GP PROVIDE CPAP MED DOCUMENTATION REQUIRED - GP CONFIRM THAT CPAP MED HAS THE DOCUMENTATION REQUIRED, IF NOT PROVIDE - GP CONTACT ME DIRECTLY TO INFORM ME THAT THE REQUIRED DOCUMENTATION HAS BEEN PROVIDED TO CPAP MED AND I NOTIFY ME WHEN I WILL RECEIVE SUPPLIES
