    My husband **** (on my insurance policy) had an appt at Premier Radiology, a location in which he has had services many times, for a CT 1/12/22. When he was called to the window by an employee and was asked for $600 payment. He explained he never pays upfront that insurance always covers the bill. She said she would bill him and he said that was fine, he would pay whatever the insurance would not pay. She had him sign the signature pad (but he could not see what he was signing). We received a bill and we paid $600 payment on 2/16/22. On 11/2/22 ********** and Blue Shield of AL said they did not get billed and suggested to request Premier Radiology submit for payment as it would have been covered 100%. I contacted Premier/Middle Tennessee Imaging, LLC by phone on 11/15/22. I called 1/25/23 but no update, so I requested again. Then called to check the status on 2/7/23, 2/28/23, and again 3/1/23. At that point it was finally submitted to insurance. On 3/9 BCBS stated it was rejected for no pre-authorization form which was required. I then called 3/16 to request reimbursement due to their error. On 4/3 I visited the ********************* and spoke ***** (supervisor) she said the girl who helped **** that day no longer works for the company. ***** said the employee had him sign the self-pay form, therefore no preauthorization. She provided a copy as well as the referral form from the primary care doctor showing we had BCBS. ***** would ask ************************* (regional manager) to escalate and call me within 72 hrs. At this time I began emailing for documentation. I emailed 4/13, 4/24, 4/25, 4/26, 4/28, 5/5. On 5/9 ********************* declined due to signing self pay form. We objected due to their employees failed due diligence. She should have looked at the referral form, insurance card, patient record. This is not our error and is unacceptable. We were told we would be contacted in 72 hrs. I included ************************* on emails 5/16, 6/6. Still no response. This is day 206 unresolved.

    I am paying my son's ****. I have an agreement to pay $10 per month. The bills come sporadically and some of my payments aren't reflected on the latest ****. States that the **** is about to be turned over to collections. Why?Something is out of order with the billing for Middle Tennessee Imaging and it is all but impossible to speak to a human.My son is incarcerated. I am the only person available to pay this **** and am doing the best that I can to pay in full.
