    I had testing at the facility in Jan/Feb of 2024. The testing was a flat fee of $99 and I informed them in February that I received an invoice from ******** for $129.15, and they agreed over the phone that I should not have received this invoice and that they would take care of the correction. I have called many times since February as I continue to receive invoices from ********., and the receptionists tell me the office manager will take care of it, but as of July, I am still receiving invoices from ********. The office manager will not call me back when I left her a message, and I've asked for confirmation from the receptionists to ensure the adjustment is made and I do not receive any more invoices. Please assist in getting this resolved. I went through the Fertility Centers of Ilinois - ******** location.

    I underwent an egg retrieval procedure on December 12, 2023. Prior to the procedure, I received a treatment estimate from the practice, indicating that my estimated out of pocket expense would be $4200, but that I had enough benefits available under my BCBS IL insurance policy's lifetime maximum to cover those costs. I understood there would be a separate $450 out of pocket cost, not covered by the insurance. That was separately paid by me.Beginning in January 2024, I received claim notices from my health insurance, indicating that the lifetime maximum for fertility benefits was exceeded during the procedure, and BCBS would be denying coverage for any amounts billed. FCI apparently failed to properly calculate my remaining benefit, or advise me reaching the lifetime maximum would leave me open to paying full, "rack rate" costs for the remainder of the procedure.Despite the quote of $4200, I then received a bill for over $9600, reflecting the amounts BCBS denied, and an additional $992 for anesthesia. I appealed the denied amounts, and those appeals were denied by BCBS. The quote was wholly inaccurate, as I am being asked to pay 250% of the original estimate. I was also asked to pre-pay $2142 for a separate procedure in March 2024, which I did. That payment was put towards the outstanding balance. I now have a remaining balance of $7680.31, and had to separately pay $992 to the anesthesiologist, who is not affiliated with FCI. I called several times to attempt to resolve this matter. I called the financial services department on 3/12 (discussion), 4/11 (VM not returned), and 5/29 (VM not returned). I am now being threatened with collections, despite my repeated calls not being returned to understand FCI's inaccurate estimate. The business has received $5942 from me directly, and a separate $25,000 from my health insurance. I wish to have this resolved by paying the difference of the $4200 estimate and my $2142 credit, in an amount of $2058.

    Everything is time sensitive and my *** appointment was not scheduled on the correct date because of a typo in an email that was sent to me. I spent A LOT of money for the *** deposit and medication needed. Not only did I take the medication that effected my hormones for no reason but they wont give me an explanation on how they plan on NOT charging me for this month's appointment and what they billed to my insurance. I spoke to an office manager and a billing manager about this. The billing manager was supposed to email me the requested documents (payments and what was billed to my insurance) over a week ago and never did.
