    On 10/18/23 I paid for in cash and in full for a AQ SPS Water Softener along with the softener and filter. The water softener agreement included delivery of 16, 40 pound salt bags. On April 2, 2024 I spoke with the secretary ***** at Culligan and told her I had not gotten the salt bags she stated that the delivery would begin on April 15, 2024. Later after speaking with the salesman *******************, ***** said I needed to sign a contract to begin delivery. I was under the assumption that the salt bags were included with the purchase. I had never been given a copy of the contract until April 2, 2024 but I was given a Gold Seal agreement for the delivery. The original contract was done on an ipad that required only my signature for the contract with my name spelled by **** as ******************. I would like the delivery of the salt bags as stated by the Secretary ***** and the correction of my name on the contract. I am only request 12 bags of 40 lbs salt since 4 were already given to me by the installation crew. I hope they can help to correct this situation so I can continue to use their services in Muncie.

    I uploaded the gold seal agreement form that was left with the customer in October.  He was supposed to sign and give to the installer if he decided he wanted to be on the gold seal plan.  As you can see the price per month is $24.95 for that plan.  Customer was never billed for the plan nor did the customer sign the agreement until April when he came into the office asking about it.  The original sales order was electronically signed by the customer and would have been sent to his email at that time.  We did print and give him a copy when he was in the office in April per request.  We would be more than glad to get the customer set up for the gold seal plan and start the delivery service but 16 bags were not included with the purchase of the equipment.  The 4 bags that were used at installation were a part on the installation fee which was part of the $2814.04 that the customer paid for in the original purchase of the water softener.
