    On Nov. 8, 2024, I paid through ISU credit union $11.82 to Union Associated Physicians, *****************************************************************************************. I still keep getting statements to pay.I've talked with billing several times with no help in settling this problem. Had trouble with a previous bill and had to take a signed copy from the bank to the Union Hospital ************** here comes the same situation.

    On March 4, 2024 I paid UAP Clinic LLC $185 for a medical procedure on 11/16/23. I paid the $185 through my online bill pay. The transaction # is 4B113ITJ. And it cleared. At that time it didn't seem that Medical and my supplement would pay for it. Then on April 11, 2024 ******** approved ***** and paid $19.47. Thereafter, *****************, my supplement paid the remaining balance due. On 6/31 I requested a refund of my $185 since they got paid twice for my procedure. I was told I would get a refund in 30 days. On Aug 9 I spoke to agent ***** who said my refund was being processed. And it would be 30 days. On October 31 I spoke to supervisor "*****" who said my refund check had been processed and I should receive it within 30 days. She did apologize for the delay, but it seems to me that if my check has been written, it shouldn't take 30 days to put it in an envelope and send it to me. I need the money and I don't know why they are dragging their feet on my refund. Can you please help? The mailing address for them is Union ************************************************

    In a letter dated Nov 7, 2024 I received an invoice dated Nov 5, 2024 with a refund check of $185, which is the amount of refund I requested.  Thanks to the BBB for helping bring this year long case to a close.  I appreciate your assistance in a quick resolution.  **** ****

    UAP and/or Union Hospital is sending me to collections over a balance they failed to notify me of. They report that they attempted to collect balances from Oct 9, 2018, February 19, 2019, and May 9, 2019 for various amounts. It is, obviously, now 2022. In this 4 years time I have continued to be a patient of UAP and pay all bills sent to my address. My address has also been the same since 2018. Not once has anyone at UAP informed me of any outstanding balances while I have been there in recent months. Nor have I ever received any bills or notices on these past due balances from 2018 and 2019. Upon calling UAP billing, they report not having any information, because once they send it to collections, they delete the records from their system. This is unethical behavior from UAP. I pay my bills, as evidenced by the last several I have paid, which have been as recent as this last month. Are they not required to give me so many notices before sending me to collections? How do I hold them accountable for this unethical management of billing and lack of informing me that I owe money?

    I am a victim of identity- theft, I am writing to request that you block all accounts disputed as fraudulent on my credit reports with Transunion, Equifax, and Experian. This information does not relate to any transactions I have made. I did not give anyone authorization or consent to use my personal information. I hereby exercise my legal rights enacted by Congress and The ************************* which explicitly states when a victim of identity-theft dispute a fraudulent account on his/her credit report it shall be honored by all credit bureaus and all fraudulent information should be blocked within 4 days and proper notification shall be given to all data furnishers pursuant to section 605b of The Federal Fair Credit Reporting Act.1.
