    My fianc passed away on May 13, 2022. I chose Brooklyn Funeral ***************************** to handle his direct cremation. At the time of my fiancs death the cause of death was not included on the original death certificate because my fianc had a sudden death. At that time I was informed by the funeral director **** that I was unable to use my benefit assignment to pay for the direct cremation as insurance would not pay out from the policy. My fianc passed away on May 13th. And although the cause of death was amended by July of last year I wasnt able to obtain an updated copy to send to my insurance until November of 2022. Once I submitted the amended death certificate with the updated cause of death I received payment from my claim but the insurance still issued payment to the funeral home. The funeral home already received payment in full for services rendered for my fianc from my fiancs son. Now Im after the overpayment because that money belongs to me. *** spoken with the director several times. The director was informed more than once from me through email correspondence to submit a letter to my insurance company on their company letterhead to state my fianc s name, DOB, policy number, the amount of the payment, and that payment was rendered in full with no balance leftover and stating to the insurance company that they are releasing the benefit assignment. In order for National Income Life to assign payment to me they need this information from the funeral home. Ive been trying to get this money since December of 2022. Please help me.

    I'm sorry for the Difficulties you have experienced during your time of loss.

    To my knowledge, once you were unable to use the Insurance Proceeds, you stepped out of the Arrangements.

    I understand the Next of Kin stepped up and took over the Arrangements.

    During the time you were actively involved we Filed a claim on your behalf with the Insuranc Carrier. Months after when the Cause of Death was amended, and a Death Certificate was submitted on behalf of the Next of Kin, the Insurance Company went ahead and processed your claim.

    I understand we offered to reimburse you upon receipt of the proceeds, however you wanted us to return the payment to the Insurance Company to reissue you a check, which is what we did.

    I apologize if this took longer than you expected, but we did return the Uncashed Check to the Insurance Carrier on your behalf.

    Hello. I understand that you have returned the check to National Income Life, however they also need in writing that Brooklyn Funeral Home and Cremation is releasing the benefit assignment. Ive spoken to **** about this issue and I have sent 2 emails as well. This is what is needed in order for National Income Life to assign a check to me. A letter on your company letterhead with my fiancs information as follows

    please attest that there is no balance and that services rendered were paid in full. You must state what the amount was and that Brooklyn Funeral Home & Cremation is formally RELEASING the benefit assignment. You must fax this directly to the claims department at  ************. This is the only way the insurance can write me a check for the remainder of the policy benefits. *** mentioned this to **** twice and sent 2 emails. Thank you.

    My mom cremation was held at this place. My brothers and I took them the Urn ( customized) so that they can put my mom inside, when it was time to pick up the Urn, they lost the plaque and the picture that goes on and inside of the Urn. When I informed them of the problem they were not compassionate At All, very dismissive, rude and carefree. They did reimburse me for the missing Plaque and picture, however when I called to see about the payment they act like they didn't want to speak they were ******* me off the phone. The ************* was very rude disrespectful and very dismissive.... Considering how sensitive this situation is to a family they handled it extremely poorly and very inconsiderable.

    The Family purchased an Urn outside of the Funeral Home, and brought it in to be filled.

    Apparently, there was a Photo and Plate, however we were not made aware of these items.

    Upon learning of theses Items we offered to reimburse the Family, for which we did.

    The complainant was not the person with whom we made Arrangements with.

    We apologize for any inconvenience.
