On Oct 21, 2022, we had a phone call appointment with our Doctor ********* Maimonides not only billed us a specialist copay but billed us $254 hospital charge and the $40 copay despite the fact that we didn't had an in person visit at the hospital. We just had a phone call with the doctor. We agreed with the $40 copay and paid it but disagree with this $254 hospital charge. We spoke with the doctor and ***** and they agreed this charge should be waived. However, Maimonides billing system won't waive it and the bill went to collections. We refused to pay it and now only is the $254 hospital charge due they also charge us another $24.46 in addition to it. We ask that both the $254 hospital charge for 10/21/2022 and the $24.46 adjustment be removed completely as it was just a phone call visit and not an in person visit. Furthermore, as we have private insurance, if in person visit, it should be in the bay parkway location and not at the hospital. The hospital are only for patients that have ****************** Hence, we should not be subject to the hospital charge and any associated fee. We already paid what we owe $40 copay.

On January 26, 2023 I went to the ** for my 3 year old daughter due to a stomach virus. We seen ******************** who informed us it was a stomach virus and that it will go away immediately. After administering an IV, she told us we could go home. Just as she left, another ******* ************************ told us that my daughter had to be hospitalized and blood work was needed. I asked what was the bloodwork for, he said to check her hydration levels. I obliged. They all informed me this was normal procedure for her virus. They admitted my daughter in Pediatric trauma care as if my daughter had surgery or a severe illness. Which was not the case. Throughout my stay, several staff including *********************, proceeded to asked me inappropriate questions about my household, my daughter and me which was unrelated to the ** visit. After being discharged the next day, I received a $10,000 bill, and found out that they performed bloodwork for a testing of ********* on my daughter without my knowledge or consent! This is illegal-over diagnosis, over treatment, and a violation of my parental rights.

On January 25, 2022, I visited my doctor ******************************* at Maimonides Medical Center for an annual physical. I provided my active insurance card so that they can file the claim with the insurer. However, when they submitted the claim, the doctor's ****** failed to code the claim correctly which caused the services not covered completely (note: annual physical are covered 100% and not subject to a deductible.) I tried many times calling them to try and get it corrected and each time I am told they are reviewing the matter. The ************* refers me to the doctor's ****** and he doctor's ****** refers me to the ************* Now the Balance Due of $200 has gone to collection. Please help, I really appreciate it.

I scheduled an appointment with Doctor *************************** for 12-20-21, before going to such appointment I asked the person who scheduled the appointment over the phone if they took my insurance ****** ********** ******, (***** ****) at the time, they said yes. I went to my appointment and gave the receptionist my insurance card she requested for me to pay $80.00 dollars as a Co-pay, which I did without any problem. I leave the ****** under the impression all is well. after a few weeks I get a **** on the mail for $386.00 claiming I owe this amount from my visit to Dr. ****** claiming they do not take my insurance and that I did not pay any co-pay. I called my insurance and they told me to call the ****** of Dr. ***** as it seems they have different tax accounts and they probably confused the billing information. I called Dr. *****'s ****** and spoke to *************************** who said she was going to look into my case. I sent her the **** I received and a picture of my $80.00 copay receipt from my visit. she claimed she was going to talk to her manager about it. fast forward another week I never heard back from *************************** about a resolution to my issue, but did received another **** for $306.00. they deducted my copay but did not **** my insurance for the remaining balance. this is an act of theft in their part and they need to be monitored as I am sure they do this to a lot of customers. I expect someone to help me in this situation as I have a newborn at home and I am in no position to pay over $300.00 for someone else's mistake. Best Regards,*********************************
