    Over the last year and a half I have been very slowly getting two root canals done on both teeth 7 and 8. after months of no return calls I stopped into his office and made an apt in person to have my crowns put on because the temporary caps are chipping away. I received my approval for the posts and crowns and upon calling to schedule that apt was informed that NYS is no longer allowing his dentistry to practice on patients that have ********** insurance. I am extremely disheartened that after all this time and pain and discomfort and self consciousness able my smile. I get to the finish line and have this happen. If there is any help at all you can provide I would be extremely and sincerely grateful. After spending four hours on the phone this morning trying to find a dentist to finish this work I have be unsuccessful and I know its just a matter of time before these temporary caps fall off. Thank you in advance for any assistance you may be able to provide.

    Please let this letter serve as response to a complaint you received from ********* ****** over treatment performed at my office on teeth #7, 8; complaint #********. Ms. ****** has been a patient at this office since 3/1/2017. On 5/11/21, ********* was seen for a routine dental cleaning and exam with the determination that teeth #7, 8 {maxillary right lateral and right central incisors) had need for endodontic {root canal) therapy followed by post and core build-ups and crowns. Request was sent for approval for treatment on the two teeth on 5/11/21 and approval received on 6/14/21 for the root canals {post and cores do not require ins. approval). The crowns on either tooth were denied at this point for an unknown reason. A re-request was done but no approval was received. 

    Regardless, the patient was scheduled for and received the root canals on both teeth on 7 /12/21. Post and core build-ups were completed on both teeth on 9/15/21. 

    Despite multiple attempts, Ms. ******'s ********** Insurance would not approve the actual crowns for either tooth until a request was made yet again on 3/11/22 with approval date of 4/14/22 (Ms. ****** having received the approval letter on 4/8/22). 

    On or around 3/17 /22, the office was notified by the Office of the Medicaid Inspector General {OMIG) of the State of NY that we would no longer be considered an accepted provider of services for Medicaid or any Medicaid-affiliated dental insurance plans. Despite our best attempts and stays of 30 days and a further 7 days following the notification, the office was unable to change the decision with the result that no payments would be received for any work performed on patients with such insurance coverage after 4/17 /22. 

    Unfortunately, as Ms. ******'s approval came right as the stay was ending, treatment was not possible on her two teeth. An attempt was made during a consultation on 5/2/22 to work around this matter and in fact Ms. ****** had seemingly agreed to the plan until changing her mind two days later. 

    It is with considerable regret that Ms. ****** decided to complain about treatment completed in a very timely matter and requesting punishment to myself and my office for something clearly beyond my control. As can clearly be noted from the above, Ms. ******'s root canals were not only completed in a timely fashion, the two post and cores were also likewise completed. In fact, the only true complaint Ms. ****** has is with the crown completion which should be directed towards her dental coverage which is obviously the sole reason for any delays:
