    I received the attached notification. Based on the reviews I have read on the BBB website and other websites, this company appears to be trying to obtain my personal information to "resolve" a charge of $890.76 (Covid test at Yes/No Covid in ******, ***** on 11/15/2021).This charge was paid with my insurance in 2021.All I want is for this company to stop contacting me. I'd also like it noted, if confirmed by your office, that this notification is not legitimate.Thank You,***********************

    Attached is the Requisition form signed by *********************** that indicates he was insured. We use two systems to verify insurance claims, ClaimMD and Marquis Logice. Both systems indicated he did not have coverage from Allied Health during the time of the specimen was taken (11/15/2021). **************** stated in his complaint that his insurance paid the claim. We have no record of payment and have asked that he provide an EOB (Explanation of Benefit) as proof payment was made.

    On March 18, 2022, I received a text message notifying me of my test results at ************* The text message included a link and a PIN number and a phone number for ************ I had not had a covid test or done any tests for health reasons in the last month, so I decided to try to clear up some misunderstanding and get the test results to the right person. So I looked up *********** on the internet and found that a business actually existed I called their number and discussed the problem.The representative I spoke to was quite unprofessional and asked for various personal information such as my birthdate and email address (to send the results of my test, which I had not had. She would not delete the record for me because she said she was a call center operator. The operator wanted to send me something, results, PCR. I declined. She explained that billing for the test often gets billed outside of insurance but if the client doesn't insurance, the client will be held responsible. My issue with Marquis Labs is that they have my name and phone number and appear to be seeking compensation for lab tests outside of insurance channels from people (someone like me) who didn't have a COVID test done. It's off and I suspect it is an identity theft scam in the least.

    After receiving this complaint, I retrieved the requisition form from the text message. The patient information on the requisition form is ********************* at ********************************************************* and the phone number is ************ dated 3/822/ and has a signature at the bottom. 

    We left **************** a voice message on March 21, 2022, but he has not returned our call to discuss.
