    Good afternoon,

The National Plus Plan (the Plan) is in receipt of a complaint filed by ********************************* on September 5, 2022.

********************** contacted the *********** on July 28, 2022 and requested a distribution application; the application was mailed to him on that date.  On August 8, 2022, the Plan received the completed application along with a copy of Mr. ********** drivers license and social security card (only one is required).  As is required under the Plan Rules, the *********** emailed Mr. ********** employer on July 28, 2022 asking for confirmation of his last day worked; a second request was mailed on August 10, 2022 (please see attached requests).  To date, the *********** has not received a response thus the distribution has not been processed.  I have emailed the employer on this date (September 9, 2022) to again request Mr. ********** last day worked.  Once we have this information, the distribution will be processed.

Further, we do not have a record of ********************** contacting the *********** for status of his distribution.  I will contact ********************** today to explain the distribution process and address his concerns.

Please contact me directly to discuss this matter further.

    Regards,
    *******************

    Business response

    09/21/2022

    Good morning,

As a follow-up to my email below I attempted to reach ********************** on September 9th by telephone.  I left detailed voicemail messages at 2 phone numbers on record for him; including the phone number referenced in the complaint.

I again emailed the employer on September 15th for confirmation of his last day worked.

********************** contacted our office on September 20th for status of his distribution application.  We explained the reason for the delay; he offered to provide his last day work however we cannot accept this information from a participant.  ********************** provided us with a contact phone number for his employer.  Attempts to reach them by phone were unsuccessful.  I again emailed the contact and stressed the importance of a response.  The employer finally confirmed Mr. ********** last day worked via email on September 20th; please see attached.

The *********** is now able to complete the processing of Mr. ********** distribution and will issue a check representing his account balance, less federal tax withholding, timely.

Please contact me directly with additional questions.

    Regards,

    *******************
