    Date : 11/24/2021 Type of Service : Bridge, Crowns I was refunded a portion of the money and the insurance was charged for services that are incomplete and my teeth keep breaking and falling out The Dentist office also sent me a letter expressing that they will no longer be able to provide me dental care or services. I would like the work to be done correctly attached is the Grievance I filed with the insurance company along with a picture of my tooth which is now in my mouth with superglue and the letter sent from the dentist office

    Business Response /* (1000, 5, 2022/04/11) */ The patient had a crown on tooth #8 that a different provider completed. We recemented the crown on 11/24/21. The patient came back because it fell out again, and at that time it was recommended for the patient to have the tooth pulled, get a bone graft, and place a bridge. The patient did have issues with the temporary bridge as the temporary did fall out a few times. We never cemented the permanent bridge, and the patient requested a refund. A refund agreement was signed and she was refunded $1103.50 on 2/03/2022. She called the office yelling because she was upset that the temporary fell out again and wanted us to complete the work. The Patient is stating that her insurance paid for the bridge, so we should complete the work. The insurance only paid toward the EXT, Bone graft, and the build-ups on teeth 7 and 9. Due to the patients irate behavior towards are staff she has been dismissed from all Mint Dentistry locations. The information concerning my crown is incorrect my tooth broke while eating some food the previous night before my dentist appointment with mint on 11/24/2022 they did not try or attempt to replace the tooth on my appointment day nor did the dentist listen when the molds were completed and I informed the tech that the molds were not correct in their response they stated I was satisfied if I was satisfied with the work why did I go to another location not only did her work come out on several occasions I had to see several dentist after that the refund request was after all the visits for the temp bridge I have the records for the dates of service and when the refund was requested I also do not agree with the Dr. signing off saying their was a break down between the provider and customer that only came after I requested the refund pictures were sent of the work for the temporary bridge so no I did not want the Dentist Dr. Whitney to complete a permeant bridge the work is horrible I sent the pictures to show how the work was done and I did complain at the time of service not to mention a portion of the bone graph was done incorrectly there was no recommendations for a crown my tooth was broke that's what brought me into the office in the first place. I did not call yelling the office manager left me on hold at for over 30 mins knowing the office was closing not only did she inform me after I asked for another location again that they could not help and the day I call I was at Modern Dental I also have phone records for the calls on that day I never yelled at anyone I did cut the conversation short after being told the had no available appointments I want a full refund services were incomplete and my tooth is still falling out the same tooth I went in the office for and is now pulling off my other tooth

    Date: 8/27/19 Cost: $1,862 Mint dentistry charged me this amount for a crown that they never put in. I have reached out numerous times to resolve this issue and they keep telling me this is how dentists work and they cannot refund my purchase. The issue is, I went to the dentist for a check up and they told me I needed a crown. I went back the next day to get the crown put in and was told it was a same day service. After they completed the core build up they then told me I needed to come back in two weeks to get the actual crown put in. I was moving out of state the next day and the staff was well aware of this. I asked for a refund and was never given one because "that's not how dentists work."

Upon checkout of the appointment we mentioned that we would be give her a call to seat the permanent once it came back from the office. The patient then stated that she was moving out of town the next day and was not aware this was a two step process. There is a script that MINT uses tow explain to patients that this is at least a two step process for crown. The patient also signed consents stating that she would need to come back to have the permanent crown seated. The patient then requested for a refund however that was denied because we did complete treatment on the tooth, and the patient was informed that she would need to come back to have the crown seated. This is not true, but unfortunately it will always be my word against theirs. The staff knew ahead of time that I was going out of town and they scheduled the appointment regardless. The staff did not explain to me that it was a two step process prior to the appointment. No one explained what the process entailed at all, so if there is a script Mint is doing a poor job ensuring it is being used. The consent forms they have you sign are stored on their computer so the patient does not have the ability to thoroughly read the consent, you are relying on what the staff state to you when you sign. The doctor who worked on me never even introduced himself or explained what he was doing. Mint treats dentistry like an assembly line which leads to these mistakes. Mint offers a free whitening tooth kit if you give them a good review which is why they have so many. If you read the bad reviews you will see many other patients having the same exact experience as mine.
