    On 1-31-2024 I went in for a PET scan and the clinic had the procedure preapproved with my insurance company that my out of pocket cost would be $220, which I paid before the procedure was started. I later received a statement showing that I owed an additional $1,121.76 for the procedure. The clinic forgot to include the injection of Gallium, which is evidently necessary for a PET scan, in their preapproval request from my insurance. That would have made my cost for the scan a true cost of $1,341.76, which I would never have approved. I had already spent $220 for an MRI so the PET scan was optional and would not have justified paying $1,341.76 for a second look. I was good with paying $220 for the second look but not $1,341.76.This was clearly an oversite of the clinic and I should not have to pay for their mistake.

    Urology Of North Texas is in volitation of sending me bills when I have a primary and a secondary insurance on file. UHC and QMB. The billed for **** services and sent me a bill for **** services and she billed for a urine and I did not do a urine, I went in to see **************** for a consult on the removal of the stone. It is against the law to send anyone a bill when they have insurance and the insurance is on file. If you will look at the previous bill the insurance paid them. On the 10th of **** the office manager at the clinic inside of medical city imputed both insurance cards. I called them and they lady stated that the bill was pending, and she did not understand why and who would send me a billing statement. I do not want any more mistakes and I want the person to examine and pay close attention to the account and send the bill over to UHC and file the copay with QMB.

    There was a lack of communication with getting my results and the collection of my urine for testing. The urine was sent to the laboratory twice and granted not to be sufficient for testing or not properly collected. The collection process needs to be explain to the medical assistant at the clinic that is receiving the specimen to be sent for testing. This increased my time for being treated for my UTI. I wont use this clinic again.
