    My mother ********************************* had eye surgery at this clinic done by ******************* on July 28, 2023. On July 24, 2023, my ***************************** dropped of some Family Medical Leave Act (****) forms at said clinic to be completed by the clinic and doctor and then to be faxed to my employer the ************* ************** ******* This **** paperwork is for me the complainant (*******************************) and my mother Eplidia so that I can be allowed by my employer the U.S. ************** to take medical leave to help my mother during her recovery. Two weeks went by and the **** notified me they had not received this **** forms . I called the clinic to inquire and was told they needed my mother&#**;s name and birthdate. Another 2 weeks transpired and the **** notified me they had yet to receive the **** forms. I then called the clinic and was told by a secretary that their tech *** needed my birthdate. Today I talked to the **** representative for the **** and was told that the **** paperwork has not been received by them.

    On July 24, 2023 we received **** paperwork on behalf of our patient's family member (son) seeking time of to care for his mother.  The **** paperwork was completed on July 25, 2023 (less than 24 hours) despite having a policy window of 48 to 72 hour of turnaround time. The **** documentation was faxed to the appropriate department.  On August 15, 2023, we received a message (by son) that the employer had not received the paperwork and needed to be refaxed to the fax number indicated. Our staff once again faxed over the information as instructed. We do not have any further documentation from the patient, son or employer that the information was not received. We will gladly provide a copy to the patient's son to hand deliver to his employer if needed. The patient has had multiple appointments with us since and no mention of the incident was reported. We will reach out to the patient that a copy of her sons **** paperwork is available if needed.
