    I went to Code 3 ER in Denton years ago back in 2017. My mom ended up paying the bill in FULL during the visit. No extra test were run outside of this 1 and only appointment so billing was accurate for 1 visit. I randomly got a credit collections for $905 last fall (2023). I did research and discover this was the only possible place this "collections" could be for. It's insane this is coming around 6-7 years after the one and only visit when again it was paid IN FULL. I disputed this on my credit report which it got removed. Then I discovered it got added again this year with a different "Account Name" but the same place and same $905. This is completely false and insane to be affecting my credit report based off one false "bill". I see multiple other people also reporting on this exact same place, how they went out of business and years later are all of a sudden getting bills that are non-existent. How can this accurately get removed from mine and countless others credit reports this is affecting.

    I got a collections notice from Central Portfolio Control last spring about a balance due to Code 3 ER but I know I paid in full the balance back when their Denton, ** location was still open. I couldnt contest it because who keeps receipts from 6 years ago. I tried to call phone#s on code3er website but all dont work. I guess they went out of business, but now I have no one to reach out to to try and get proof I paid off my balance. I reluctantly paid the collections notice. Some time later, Im cleaning out old stuff and I find the bill and receipts that I paid my balance. I noticed the account# on the receipt is one letter off! I was young back then and didnt think to double check, I just trusted the receptionist applied my payment to the correct account considering I brought the bill in person to pay. Now 5-6 years later I get a collections notice! All because the receptionist didnt apply the payment to the correct account and now I have no one I can reach out to even try to correct this. Amount in dispute: $352.26

    12/23/2018 Code 3 ER, Rockport *****. Went for infections. I paid my deductible $100. They billed insurance $500. Was paid insurance allowed of $396. Never heard from them again. In 2019 Code 3 ER closed their doors. In 2020 Code 3 ER went into litigation and filed bankruptcy.Then in 2021 and 2022. A collection of $1300 was put into my TransUnion Credit Report. I filed a dispute showing my information. Dispute was in my favor both times.10/19/2023. **************** Ser has reported to Equifax the same bill. Now I have to waste my time disputing something that was already decided in my favor through ************

    Business/medical facility pre maturely reported they were in network with my major provider at the time of service in which they were not and eventually ended up closing the location I received services due to poor and I believed illegal billing practices leading to a high collection bill that was unfairly sent/placed on my credit report

    This company contacted me on February 16,2023 about "unpaid " bills from 2018 and 2019. When asked where these came from, they stated something about the company closing down due to litigation etc etc. They say they tried to call me but do not have the correct phone number for me . I question the validity of the bills and why they are popping up 4 and 5 years later. And the phone number to call belongs to ***************************** and not Code 3 ER.

    BBB unable to locate business
    I went to Code 3 ER in ******** on December 15, 2018. They attempted to bill my insurance twice (for far too much) and then did not bill me until December 14, 2019 and promptly sent me to collections. This is a direct violation of the timely billing law in Texas which states they must bill me by the 1st day of the 11th month. I had not done anything about it before now because I didn't know my options but I knew what they were doing was fraudulent. Now I'm being unfairly harassed for years by collections. This should never have gone to collections.
