- Additional Info:On May 14, 2002, a plan for reorganization of the company was confirmed by the U.S. Bankruptcy Court in Dallas, Texas, under Chapter 11of the federal Bankruptcy Act, case # 02-34243-RCM-11. This permits the company to continue to operate with court supervision until further notice. Those with monetary claims against the company are advised to obtain a Proof of Claim form by writing to Clerk of the U. S. Bankruptcy Court for the Northern District of Texas, Earle Cabell Federal Building, U.S. Courthouse, Room 1254, Commerce Street, Dallas, TX 75242. Proof of Claim forms must be received by the Debtors' claims agent, Logan & Company, Inc., Attn: Senior Living Properties, Claims Processing Department, 546 Valley Road, Upper Montclair, N.J. 07043. The case number should be entered on the form. (Claims must be submitted by October 31, 2002.)
