    I have a flexible spending card through my employer that I used at the Velocity urgent care in ************ **. The receptionist informed me that I had a past balance of ****** which I gladly paid. She could not tell me what date of service it was for and told me to contact billing. Apparently they outsource their billing department. I have contacted them of several occasions to request an itemized billing statement for the payment of ****** and each time the csr tells me that the statements will be sent to my email within 4-5 business days. I have nothing and now my FSA card company is requesting I repay the ****** for failing to provide this statement that shows that the services were approved services.I paid ****** to them on 4/6/23 and have yet to receive a statement for what I have paid for. I requested all statements from last year to present to see if any of those would say I owed ******. I am getting no where and want to be refunded what I paid since they cant show me what I paid for or for them to provide me the statements requested.

    My name is ***************************, and I am the Revenue Cycle Specialist here with Velocity healthcare.  Let me begin by apologizing for the lag in our communication with you receiving your itemized statement.  I know firsthand how the *** works, and how they request documentation so you can be reimbursed fully.  The date of service that you owed $169.00, was for 10/26/2022.  Initially Virginia Premier denied the office visit, which was $170.00 because the visit was not authorized.  You had a credit of $1.00 on a date of service of 01/24/2019, and that credit was applied to your account for date of service 10/26/2022.  And you made a payment of $169.00 on 04/06/2023.  Attached you will find your itemized statement that gives a full breakdown.  If you have any questions, feel free to contact me personally.  Again, I would like to apologize for any inconvenience we may have caused you.
