    On November 18, 2023. I went to see ****************************, at Optimum Care, LLC. post-accident to receive treatment and care for a lower back injury that might have gotten aggravated at the time of accident. I called Dr. ******* office to setup an appointment and I was told to just walk-in and signup with the receptionist at their office. When my turn came to be evaluated, I walked into the Dr. ******* *******ation room for what turned out to be a 20 min time duration from start to finish *******ation. During this short 20 minute *******ation, I was asked questions pertinent to the reason for my visit, and received a shot (one shot only) of Ketorolac for pain, along with 2 pills of Dexamethasone for inflammation, given to me by the nurse to take orally during Dr. ******* and macule supporting belt. I did not receive any other medications during my Dr. visit / ******* or received any prescriptions from the Dr. himself, nurses or any other staff during my short visit/ stay.From my lawyer (******* retained for accident representation) apparently received a similar Invoice / Statement with items repeated multiple times, items included for invoice that were not provided or delivered. When I requested the a copy of the Invoice I was apparently no longer dealing with Optimum Care, LLC., but dealing with a billing center that works directly for ****************, on my copy I had different amount invoiced as my lawyer for the amount received and duplicated in the invoice. It was also noticed that my copy arrived with an additional claim for the same event / day an occurrence were the entire visit was re-added and items were now showing 4 to 6 times added to further pad the invoice.After I left the office, I was called by their staff to pick up a prescription with them which I refused to do, since my experience for the short visit was already not pleasant. I did not get any further calls after that for prescribed medication. I promptly filled-in a customer experience survey post my visit and it was not a favorable one based on my experience and treatment received at Dr. ******* office.the attached files are the copy of the bills i have received.

    Please be advised that **************** agreed to reduce his fee to $3,000.00.  Accordingly, effective immediately, I am hereby withdrawing my complaint against him.

    I ,************************************** was seen at Optimum Care LLC on 10/21/2021 for sinus congestion, headache and a cough and low grade fever. After 45mins of waiting I was asked to pay all fees up front in the *** of $150.00 which I did ( I have my bank statements as proof. Because I had no insurance.**************************************, after some questions and ask of I santed a Covid test - I declined. I informed him that this is my usual process and I am a RN and just need some Antibiotics. He stated that I was not going to tell him what to write for. He then asked me if I wanted an steroid injection and stated for what and declined. He wrote for an antibiotic and I left. Then today I get a **** for $1,220.00 with cost for several things not done or received. This is a fraudulent ****/ claim. Noted on the **** is the $150.00 that I paid.My total time in the office was less than 15mins face time with this Doctor ******* ensure as per your protocol as this is shocking , an a hurtful attempt at usury and fraud.**************************************
