    We went to ***** Achatz for our 2021 tax preparation. He stated we had $274 that was to be REFUNDED to us. Our financial advisor reviewed the return and stated that our amount that we withdrew from our *** to go to our **** *** was documented wrong on the 1040 form as a conversion versus a withdrawal. We went back to ***** and he amended the form where we went from getting $274 back to paying $6,576. He told us that he was going down to the post office himself to mail the amended copy. After months of no communication from the ***, I called the *** myself, I was on hold FOR 2 HOURS only to find out that they never received the amended copy. We had sent the check to the *** THINKING that the amended form was sent. After sending the $6,576 check, months later we received a letter from the *** along with a check from them stating that they had no idea where that amount came from. We went to ******* office to get all the amended copies of the tax return so that WE could send the amended copy of the return ourselves. I spent hours on the phone with *** to see that they received the amended copy and they mentioned that it was going to take a few months for them to process it. On October 1, 2024 it was finalized and had to pay $7,674.51 with the interest, penalties and fines. That is $1,098 more than we would have had to pay if it was done correctly. The state return: We went from no refund, to after amended copy to paying $946 and then the state sent us a corrected copy and ended up paying $3,723.86 which included $648.88 of interest along with $47.00 underpayment interest. So after 3 years of working with correcting HIS MISTAKE, we ended up paying $1,794.00 in fines and interest. We are requesting compensation on the amount that we ended up paying in fines, penalties and interest on this frustrating and costly issue along with the headaches and time spent running to his office and the hours spent on the phone with ***. (Cost $150 for prep as well as $1,794 in fines)
