    I had booked a COVID-19 Booster vaccine from the government site myturn.ca.gov for Nov 24, 2021. At AFC Urgent care Newark located at **** ********* ****, *******, CA *****. I went on Nov 24 got the booster shot. After that they sent a Bill for $832 to Unifier Health Care claiming I went for Doctor visit, details of the visit reason not there. This is a case of fraud, how these private operator is billing customers for doctor visit. I have proof of the appointment & bill

    My husband, son and I went to AFC and signed paperwork that stated they charge $55 for COVID testing for travel on Jan 27 (son) and Jan 28 (my husband and I). In March, all three of us were sent a bill for $125 each for hcpcs code ***** - global fee for an urgent care PLUS the $55 for the covid test. Not one of us went in to a room or saw a doctor. They took us in the lab, swabbed our nose and had us wait outside. I am not sure where this charge is coming from since the paperwork clearly said there was only a $55 fee which I verified with the receptionist prior to filling out the paperwork. I called the ************ phone number and spoke to Peter who said that is their fee for the urgent care. I explained the situation, the use of the HCPCS code and since we didn't get put in a room or see a doctor I was unsure what this fee was for since I asked the receptionist the actual cost of the test. He said he would have someone review it and to call back in a week. I asked for a call back but he stated to call back. I am thinking I am NOT getting a call back. I want this resolved!

    I keep receiving a bill in the mail about a covid test that is always paid by the VA at this facility. I have called and can never get anyone to see why am I all of a sudden receiving bills when the VA always pays this. This needs to be rectified immediately before it goes on my credit.

    AFC overcharged me for a school physical and 2-step TB Skin test. I have called multiple times and provided by email the proper paper work to prove my case. I never received any of the money owed nor any follow up communication. This is aggravating and makes me wonder how many other people are being taken advantage of to the point that CMS may need to look into this business for further investigations.

    Had a work related injury that was to be filed under Workers Comp and billed to the employer in Jan of 2021. Was seen several times with them just having me come back on set dates in order to keep billing with no resolve to injury (seemed to be milking the system). Couple of the visits they billed correctly. Then they decided to start billing my VA/TriCare insurance. Then they started billing me directly on others. Contacted them providing the information. Even went to the treatment office 45 minutes away (one way!). Was told it was taken care of. Then all of a sudden the ones billed to me was billed to the VA/TriCare. Called again and pointed out they were again billing incorrectly. This drug on till Nov of 2021, when I thought it was all taken care of. Now as of today 3/28/22 I received a bill in the mail dated for a visit on 3/4/21 (over a YEAR later!). Note on the bill even states I had not received any bills since Nov 2021 (when it was suppose to be resolved). I called and first time after being on hold for 7 minutes the system kicked me off hold. Called again and after being on hold for another 5-6 minutes was providing details to the lady and she hung up on be at exactly 5pm. Called right back only to be on hold for 2-3 minutes and being sent to voicemail. I assume they ALL clock out at 5pm and hence why I was hung up at exactly at that time and why I went almost immediately to voicemail after navigating their phone system menu. I've spent well over 6hrs of my own time in the past 12 months trying to resolve THEIR screw up. It can't be that hard to get this straight on their end, unless it is done on purpose in the hopes the person will give in and just pay to make it go away.

    Date of the Transaction was July 8th, 2020. I went to AFC Urgent Care for what they told me was a free Covid test. I contact you late because when they, AFC, got back to me was months later and only because they submitted a bill to collections for a charge concerning the free Covid test. Apparently they want to charge for the reading of the results and they messed up on billing the VA which is my only medical coverage. Well they shouldn't bill me for one, they said FREE Covid Test and two, they never got back to me to read me the results. They claim the charge is for the reading of the results. The company, AFC, never contacted me with the results. So they want to charge me for a job they never did. They they never got back to me with the reading of the results of the Covid test, and they want to charge me because they did not process the bill, which should not even exist, as they quoted, Free Test, and never said anything about there would be charges. They, behind my back, tried to bill the VA for this, but they never used the right codes and were unable to charge the VA. So they sent the Bill to collection. I ask the Collection Agency what the Bill was for. They said it was for the reading of the Test results. They have never got back to me to tell me the Test results. So to bill me for the reading of the Test results is to bill me for something they never did. If this is reported to the credit bureau it is reporting a lie. Please help me with this horrible business practice. Thank you. I do not have a copy of any bill.

    On 1/13/2021 I went to AFC (American Family Care Urgent Care) for a Covid test. They wouldn't perform the test without me paying for the charge of $175 even though I told them it was fully covered under my insurance. They told me I needed to pay for it and then they would bill my insurance (United Health Care) and once they pay them they would reimburse me. I have since called both AFC and United Health Care and no claim has still been submitted by AFC to my insurance. I have called 8+ times to get this taken care of and every time I speak to someone they tell me it will be submitted and when I call my insurance nothing has been processed. The front desk person at AFC who made me pay for this charge was "*****" who claimed that they can't trust that my insurance will pay them and that is why they made me pay in advance. And the AFC Manager of the location is Daniella Nutchio who I have called and left messages for at the front desk and she never returned any of my calls. Even when I call when she is in office they tell me she is busy.
