Multiple appointments on 2/1/2024, 2/6/2024, 2/7/2024, and 2/13/2024 Misdiagnosed 3 times before receiving proper medical diagnosis and treatment Was invoiced for all 4 appointments and was told that only one appointment will need to be paid and the other three would be absolved on May 2nd 2024. Kept getting invoices for all four appointments after May 2nd and called CVP Physicians/CEI multiple times to get the invoices closed out and every time someone in billing said the invoices would be taken care of. Each time, nothing happens and I get invoiced for all of the appointments again. The last called I made to CVP/CEI, the lady in billing said she would pass this information to a billing supervisor, but she could not guarantee that anything would happen because situations like this have occurred in the past and no action was taken by a supervisor. I am more than willing to pay the invoice for the first or last appointment immediately, but only when I get an updated invoice for one appointmentInitial Complaint

This company seems to almost be hidden so here is what I can tell. In October 20, 2023 I saw Dr. P*** at the practice of CVP. They were given both my primary and my secondary insurance. My secondary is ***** Medicare which this company is in Network. The Executive Office, *******, of ***** has contacted this company multiple times as well to tell them that their determination was denied. I have contacted this company. My wife, ***, has contacted this company multiple times as well. The last message that ***** received from Patricia at ###-###-#### is that she was not accepting the determination of ***** and that (basically and paraphrased) she was making certain we paid. This has just escalated to this point in the last couple of months and we realized we have an issue. My wife last contacted them today and the Supervisor who was suppose to call back after lunch (Latricia) did not. I have attempted to contact he Corporate Office as well.Initial Complaint

I went in for a scheduled eye exam, and I provided my health insurance card. The people at the desk submitted my information and never mention the word about my insurance and proceeded to allow me to get this examination. Months later I get a bill $640. I called this office which is the one in Fairfield where I have my appointment. I spoke to a manager or clerk and asked her that why was I getting charged this amount. She told me that they did not accept my insurance. I told her that I should’ve been informed that way I can make a decision whether to proceed with the appointment or not. She then told me that it did happen one time before with another patient. I told her that this is unacceptable and they cannot just Make a decision for me without letting me know about my insurance, not being accepted.Initial Complaint

I would like reimbursement for 60 dollar paid last year for a copy of my test results prescription. They are collecting monies for test results or prescription for eye glasses even though they have been paid by the insurance carrier and the test result or prescription for glasses should be issued with out an additional charge. They are doing this to everyone and I would like for all the people who were charged for test result and prescription to be refunded since they first began charging people for test result or prescription. They will attempt to say that my services were different because it was a diabetic exam but per last year they charged me for the prescription.Initial Complaint

Issue 1: On October 18, 2023, I was given an oral estimate of $993 for surgery. The estimate was for surgery center charges at *** ********* Hospital AND CEI services. A written notice was mailed within a week. After insurance negotiations, the actual amount billed was $1,015 from CEI and $4,446 from *** ********* Hospital for a total patient responsibility of $5,461. There were no complications with the procedures. When the bills came in I contacted the hospital. They acknowledged that CEI provided an inaccurate estimate and agreed to reduce its bill by $2,223. I also contacted CEI. They acknowledged that their estimate form was misleading but refused to accept financial responsibility for the inaccurate estimate. Desired resolution: $2,223 payment from CEI to me Issue #2: CEI accepted three payments for its services: $993 in October by credit card, $993 on November 1 by credit card, and $1,015 in November from my insurance company. The second payment was credited back to my credit card. CEI claims that the “check is in the mail” for a partial refund, but it has been more than a month since the overcharge occurred. Desired resolution: $993 payment from CEI to meInitial Complaint

On Wednesday, Dec 13, 2023, I had a scheduled appointment with CEI/Dr Daniel H***** at the ***** ********** ** office. This appointment was scheduled following a previous visit approximately 6 months prior. On the Sunday prior to my scheduled appointment, I received a phone call (automated) reminding me of my appointment.l When I arrived for my appointment (15 minutes early), I was told the doctor wasn't there. No apologies or explanations, just the fact the the doctor was not there. The receptionist who informed me of this was not caring or concerned about this at all, just informed me that I was out of luck. This is the second time I was treated in this manner at this location of CEI. I was then told to re-schedule the appointment, which was 6 months in the future. This sort of treatment never happened in the 20 or so years that I had Dr. B**** as my ophthalmologist.Initial Complaint

I had an eye procedure in May 2021. I was evaluated by Cincinnati Eye Institute for the necessary surgery and was also vetted by Cincinnati Eye Institute insurance / financial coordinator to verify if my insurance would cover the procedure. About a week after the doctor consultation, I was informed by the Cincinnati Eye Institute financial office that my insurance company approved the procedure. So they surgery was done in 2021. But, as of May 2023 I have been receiving a bill form *** Physicians / Cincinnati Eye Institute for $1,500. I do not own this money since *** Physicians / Cincinnati Eye Institute said they got approved by my insurance company and they performed the surgery under those conditions. . I called *** main billing line on 6/7/23 & 6/15/23 and talked to Tony to resolve this issue. He said he would send it to his supervisor Demetrius.. I never heard back from any one. I called again on 6/23/23 and spoke to Trisha with no resolution. I called again on 7/20/23 and ask to speak to supervisor Demetrius. I got his voicemail and left a detailed voicemail. I got no response. I called again on 7/21/23 left another message for Demetrius and again no response. So I called the main billing line again on 7/21/23 and spoke to Tony again. He again said he would send another message to his supervisor Demetrius. it is 7/24/23 and I have still not gotten a response from anyone to resolve this issue. I do not own the bill. The surgery was done in 2021 and they are sending me a bill 2 yrs later. I no longer even have that insurance coverage that I had in 2021 . I want this resolved and cleared that I do not own this bill, so it does not effect my credit score.Initial Complaint

I visited this business on two occasions, 9/13/22 and 2/20/23. I specifically asked at each appointment if my ***** ******** would cover the charges after paying my copay. I was told yes my insurance would cover the charges. Six months after the first visit I started receiving bills for charges not covered by my insurance. I called their billing department. I spoke with Hanna 3/14/23. She said they’d resubmit to *****. On 4/4/23, I received another bill. I called and spoke with Hanna again. She said she’d speak with her supervisor. Didn’t hear anything back. Received another bill dated 4/20/23. I called and spoke with Tony. He said they’re not responsible for telling me what charges are not covered by *****. I requested a call from his supervisor. I’ve heard nothing. First, if they didn’t know what was covered by my insurance they should’ve explained that up front. Second, sending me a bill six months after the first appointment is some of the worst accounting I’ve seen and I’m seventy years old. Third, their customer service is deceptive and lacking on follow up in every area except taking money from a client. These charges need to be reversed and the balance should be $0.00Initial Complaint

To Whom May Concern, I got the final notice letter of my outstanding balance $1,290.67 in my account on 1/6/2022. I called the CEI billing dept. **************. I was told my balance is $1,302.16 + $1,016.00. I had my left eye floaters checked by a personal eye doctor. My eye doctor referred me to the CEI to do laser surgery to repair the tears induced by my left eye floaters on 11/10/2020. The CEI scheduled me for a follow-up check-up in two weeks on 11/24/2020 after the first surgery. I asked why the follow-up check was needed. I was told that the follow-up check was to make sure the laser surgery was successful. I did not initiate my second eye doctor's visit. I was supposed to have a follow-up check but got the second surgery on 11/24/2020. I was shocked to get the repetitive charge on the second surgery. I did not agree to pay the charges of $519.49 (physician) + $508.00 (surgery center) for the second surgery because the operating doctor told me so and the second makeup surgery fixing the same eye and same issue due to first incomplete one. This is why I have not made any payment. I agree to pay the charge $263.18 (office) + $519.49 (physician) + $508.00 (surgery center), for which in total $1,290.67 from the CEI final notice letter dated on 1/6/2022 rather than $2,318.16 in my online account. I hope this matter can be resolved for mutual interest. Sincerely, *******
